If you are requesting medical records related to services you received at Counseling and Psychological Services (CAPS), please note the following:
- Please call our office and request to speak with the most recent clinician with whom you met. If that clinician is no longer available or you do not remember their name, please request to speak with the Associate Director, Clinical Services at CAPS.
- You must fill out a Release of Information Form at CAPS before records will be provided.
- CAPS will respond to complete/authorized requests for medical records within approximately seven (7) business days after receipt of a valid request.
