• Return the form to Counseling and Psychological Services by either bringing it to Student Union 1, Room 3129 or faxing it to (703) 993-2378 along with a copy of a picture ID. If you fax the release, be sure to have your signature witnessed and to provide accurate contact information for all parties involved.
